Creating A Contact Form Within an Article

I am trying to create a contact form within an article or module.

I've created the contact form component, however I can only get it to load through a menu, which is not what I want.

I want to have the form at the bottom of my contact page http://www.henryweaver.co.uk/index.php/contact

I'm wanting to achieve something like this.

You can create 'bespoke' contact forms using this component

Activating the Form option allows you the create a contact form in an article
