Click on the team and then click on the button at the bottom for MAKE A BOOKING
As you'll see see in phone mode the form does not move.
Is there a piece of code or someway this can be made to work?

Hi Terry

Checking on an Android device your form appears to be working without issue although is does appear rather small on the screen. I am not familiar with this extension so I would suggest contacting the extension developer to resolve this issue.
